when i was about 6 years old i had friends that lived on a farm. we would play at their house nearly every day, and one of the games was playing family. it was my turn to be the kid, and "my parents" sent me to get water. i slipped on some black ice and crashed my chin into a corner of the fountain. The pain was damped down by the cold, and so i thought it was just a bruise. When i got back into the house, my friends were all like, oh my god, you're bleeding! then i passed out.
